Abstract:

We would like to present a rare case of hepatitis with Fitz-Hugh-Curtis syndrome caused by Chlamydia Trachomatis infection in a middle-aged man.

Perihepatitis or Fitz-Hugh-Curtis syndrome is a well-known complication of Pelvic Inflammatory Disease in female patients, but it has only been described in a handful of cases in male patients. It is normally characterized by right upper quadrant pain caused by string like adhesions between the liver capsule and parietal peritoneum. Hepatitis on the other hand has never been reported due to Chlamydia infection.

The patient presented with three-day history of right upper quadrant pain and mildly deranged LFTs. US and CT images revealed a thickened gallbladder wall with small calculi and slightly heterogenous mildly fatty liver; therefore, a decision was made to take him to theatre for laparoscopic cholecystectomy. During surgery it became evident that the whole liver was inflamed with purulent exudate and adhesions were noted between the liver capsule and the parietal peritoneum. We decided – after intraoperative consults with Gastroenterology and a fellow General Surgeon – not to proceed with the planned operation.

His viral screen confirmed Chlamydia Trachomatis infection via PCR testing, he was treated with a course of Doxycycline and Metronidazole and his symptoms settled with the above management.

Dr Eszter Tothne Karip has completed her Medical Degree in Budapest, Hungary and has since worked in various hospitals across Ireland, United Kingdom and Australia throughout different specialities including Intensive care, General Surgery, Trauma & Orthopaedics, Orthogeriatrics and Medicine. She is currently working as a surgical registrar in Maitland Hospital.
